Social Work Assistant - Adult Social Care

Job Introduction

We are looking for a Social Work Assistant who enjoys working to achieve positive outcomes for adults following an admission to hospital or a period of reablement or rehabilitation.  The successful candidate will have the opportunity to work within a variety of professionals including Physiotherapists, Occupational Therapists and reablement and rehabilitation assistants, to support individuals to achieve their maximum potential and enable them to live independently.  If you like a challenge and want to develop your assessment skills and gain experience working with adults, then this is the role for you. You will be supported to develop your assessment skills under the Care Act 2014 and will have the opportunity to work with individuals and their families/carers using a strengths-based approach to promote independence and minimise risk.  The ideal candidate will be someone who has experience of working with people within a health and social care capacity.  The candidate will have excellent communication skills and a desire to work with people to make a positive difference to their lives. 

 

You will be joining a supportive team which includes social workers and social work assistants to assess people to ensure their timely discharge from rehabilitation, recuperation and reablement services.  The safety of our team and the people we work with is paramount and therefore we adhere to infection control measures and have a flexible/blended approach to smarter working.  The role is based in the community and includes assessing people in bedded units and, in the individuals, own home.  The successful candidate will have completed a Diploma 3 or equivalent in supporting people.

Main Responsibility

  • Undertake strength’s-based assessments as outlined under the Care Act 2014 for service users and their carers
  • Supporting service users and carers where necessary in meeting agreed outcomes in the most resourceful way.
  • Be able to use the council’s email, social care data base and electronic recording system and any other systems deemed essential to the role.
  • Be aware of quality standards and performance measures and ensure they are adhered to
  • Provide Practical Support where needed to individuals. 

The Ideal Candidate

  • Diploma 3 or above in Health and Social Care/Supporting People or equivalent
  • Have experience working in a social/health care setting
  • Hold a driving licence and have access to a car available for work
  • Good communication skills with individuals and their diverse needs
  • To work as part of a team including working co-operatively with other professionals and agencies.
  • Awareness of adult abuse and Mental Capacity.
