Job Introduction

We are currently seeking experienced social workers who have knowledge and understanding of working with adults who have recently been discharged from hospital. The ideal candidate will have experience and skills in a strengths-based approach focusing on improving individuals’ independence. The work is fast-paced, short term and varied and as a social worker you will be providing support to a variety of discharge to assess services. This includes assessing people in recuperation and rehabilitation bedded units and, in the individuals, own homes.

The candidates will have the opportunity to work with a variety of health and social care professionals to ensure that individuals potential is maximised, and to promote the combined ethos to get people home. You will be joining a very supportive team that seeks to promote individual and the team’s development. We have an avid focus on reflective practice and learning outcomes and carry this out in a variety of different ways including monthly continuing professional development sessions.
Main Responsibility
- Undertake strengths-based and person-centred assessments as outlined under the Care Act 2014 for individuals and their carers.
- Evidence a sound knowledge and application of legislation relevant to role including Care Act 2014, and Mental Capacity Act 2005 etc.
- Work collaboratively with professionals across health and social care to holistically meet individual’s needs.
- Manage a case load with guidance from line manager and prioritise the competing demands and time pressures of ensuring there is continual flow through the discharge to assess services.
- Complete safeguarding enquiries, mental capacity assessments and best interest’s decisions with guidance from line manager.
The Ideal Candidate
- Social work qualification (DipSW, CQSW, Degree in Social Work or equivalent).
- Registered with Social Work England
- Safeguarding Adults Investigators training or working towards
- Will need to be able to drive (or have a driver) and have access to a motor vehicle.
- Excellent verbal and written communication skills
- Be able to make decisions and demonstrate management of risk using evidence-based practice.
- Ability to work as part of a team including working effectively in partnership with other professionals and agencies.
- Be able to demonstrate flexibility, willingness and emotional resilience to continue to develop as a professional and be a strong advocate to support people with change.
