Social Worker - Hospital Discharge Team

Job Introduction

We are looking for Social Workers who want to join a supportive team, that is proud to work together with our community and hospital health colleagues, to meet the needs of the people living in Milton Keynes.

Our social workers priority is to support people to return home and recover from a period of illness by avoiding long stays in hospital which can result in their health and well-being deteriorating. We achieve this by making use of the short-term services we have available to aid discharge; we are fortunate in Milton Keynes that we have several options to meet people’s needs when leaving hospital. The social workers role will be to assess people receiving these services to ensure care and support plans are put in place building on the person’s own strengths and assets along with formal support if needed.

As a team we are committed to the development of our social care professionals and our team. So, if you are looking for a new role where you can expand your assessment skills, work together with health professionals and progress as a social worker then this is the team for you.
