We’re seeking an experienced and enthusiastic social worker who understands NHS Continuing Health Care eligibility, assessments, and processes. You’ll work with NHS Continuing Health Care colleagues to ensure that Milton Keynes residents get the support and funding they’re eligible for.
You’ll manage a complex caseload and support the wider adult social care teams with NHS Continuing Health Care processes and assessments. You’ll also be involved in dispute and joint panel funding decisions, so you’ll be confident about challenging decisions to ensure the appropriate support or funding is agreed.

A social work degree or equivalent is required, and as some travel around the city is required we expect you to be able to drive with access to a car.
Main Responsibility
- Undertake assessments as outlined under the Care Act 2014 and NHS Continuing Health Care Guidance. Work in partnership with colleagues external to social care to holistically meet individual’s needs.
- Work in partnership with NHS Continuing Health Care colleagues to holistically assess individuals needs and positively challenge eligibility/funding decisions.
- Complete safeguarding enquiries, mental capacity assessments and best interest decisions.
- Manage a complex case load, manage the competing demands and time pressures of completing NHS Continuing Health Care within the 28-day time period
- Support other colleagues across adult social care with Continuing Health Care cases and processes.
- Attend and contribute to dispute and joint funding panel processes to advocate for health funding for the individual.
The Ideal Candidate
- A social work qualification (DipSW, CQSW, Degree in Social Work or equivalent). Post qualification award (e.g., AMHP status).
- Registered with Social Work England.
- Experience of being a Safeguarding Investigator.
- Experience of managing complex cases, working autonomously and have sound evidence-based decision making.
- Excellent verbal and written communication skills
- Ability to make complex decisions and demonstrate management of risk using evidence-based practice.
- Able to work positively with health colleagues to ensure the appropriate outcome is achieved for the individual.
- Can evidence a sound knowledge of NHS Continuing Health Care, Funded Nursing Care, Fast Track CHC and the Care Act 2014.
- Able to demonstrate flexibility, willingness and emotional resilience to change and be a strong advocate to support with change.
