Social Worker Community Support Team

Job Introduction

We have an exciting and unique opportunity for two experienced Social Workers to join our Community Support Team. The team is a joint initiative between Housing and Social Care, working with individuals who have a variety of challenges such as self-neglect, hoarding and will be initially unwilling to engage, and issues related to alcohol and substance misuse.

Ideally, you will have at least 2 years post qualifying experience and be able to think on your feet in a crisis. This is a challenging but very rewarding job where you can make a difference to support someone to help them change their life. 

Main Responsibility

  • Assessing service users needs and providing solutions of how we can help them better their lives. 
  • Ensure that any interventions or services support the service user in way that enhances their independence, choice and wellbeing
  • Monitor and review support plans and services in place to ensure that eligible needs are met in a way that the service user has determined
  • Balance the varying demands of a complex social caseload, maintain accurate individual case records and ensure that all required reports are completed.
  • Participate in supervisory meetings  with team manager and/or a senior member of staff on a regular basis ensuring that the line manager is adequately informed about cases and practice , be responsible for own development and identifying training needs. 

The Ideal Candidate

  • A qualified social worker registered with Social Work England and hold relevant experience within field for over 2 years. ​
  • Someone who enjoys talking to people and trying to put things right. ​

  • Someone  who works well in a team

  • Someone who is flexible  and can cope with working in unsanitary conditions when required, PPE will be provided. 

  • Someone who can see the potential of the people we are working with and have the ability to help them reach their potential.

  • Ability to analyse complex information and the ability to manage complex workload.
