Social Work Assistant

Job Introduction

Our friendly and supportive Older People’s Assessment Team is seeking a caring, common sense Social Work Assistant to help local residents live rich and fulfilled lives.

You’ll work alongside Qualified Social Workers and Support Workers to provide high quality personal care to people 65 years and over, sometimes with complex health needs such as dementia, or in complicated circumstances.

Our team is based in the community and the support we provide runs from initial assessments to the provision and review of short and long-term care. We also cover safeguarding and mental capacity assessments, investigate concerns – such as if someone is self-neglecting - and help people to access housing where needed.

You’ll assess people receiving our services to ensure care and support plans are promptly put in place that build on the person’s own strengths.

Main Responsibility

  • Carrying out strengths-based and person-centred assessments as outlined under the Care Act 2014 for individuals and their carers.
  • Duty responsibilities and carrying out welfare checks.
  • Applying legislation relevant to role including Care Act 2014, and Mental Capacity Act 2005 etc.
  • Working together with professionals across health and social care to meet individual’s needs.
  • Manage a case load with support from line manager.
  • Complete, mental capacity assessments and best interest’s decisions with guidance from line manager.
  • Balance the varying demands of a caseload, maintain accurate individual case records and ensure that all required reports are completed to agreed timescales.
  • Can recognise risk.  Know when to escalate, contribute to writing plans with guidance and support.

The Ideal Candidate

  • Previous/current experience is social care.
  • Understanding of safeguarding adults and when to raise alerts.
  • Able to drive and have access to a motor vehicle.
  • Excellent verbal and written communication skills.
  • Ability to make decisions and demonstrate sound risk analysis.
  • Can work as part of a team and with other professionals and agencies.
  • You’ll be flexible and want to develop as a professional and empower others.
