Our friendly and supportive Older People’s Assessment Team is seeking a caring and experienced Senior Social Worker to help local residents live rich and fulfilled lives.

You’ll work alongside qualified Social Workers, Social Work Assistants and Support Workers to provide high quality intervention to people 65 years and over with variable care and support needs.

About the Older People’s Assessment Team

The Older People’s Assessment Team uses a strengths-based approach to support people to continue to live at home, or close to home. We endeavour to achieve this through the promotion of an individual’s choice and control, providing diverse services and enabling them to live as safe and independent a life as possible.

We aim to deliver exceptional services under the Care Act 2014; Mental Capacity Act 2005 and relevant statutory guidance. Our approach promotes the persons independence and choice while meeting their care and support needs.

Main Responsibility

  • Operate within a sound understanding and application of current legal frameworks including the Care Act, Mental Health Act, and Mental Capacity Act.
  • Provide professional leadership in cases and support unqualified workers in their practice.
  • Stay abreast of developments in service delivery, best practice, and case law to ensure high standards of service.
  • Assess the needs of adults, determine outcomes in partnership with the individual, and promote independence and well-being.
  • Monitor and review support plans to ensure they meet identified needs.
  • Maintain accurate case records and complete required reports within agreed timescales.
  • Participate in regular supervisory meetings to ensure effective communication and informed decision-making.​

The Ideal Candidate

  • In-depth knowledge of legal frameworks such as the Care Act 2014, Mental Capacity Act 2005, and the Mental Health Act 1983.
  • Post-qualification experience as a Practice Educator, Best Interest Assessor or Approved Mental Health Professional is desirable.
  • For this role you are required to have a full UK driving licence and provide evidence of your vehicle insurance with business cover and use of a vehicle. If you are successful, you will be required to supply evidence of your licence prior to starting the role.
  • Qualified Social Worker registered with Social Work England.
  • Should you be offered the role, as part of the pre-employment checks an Enhanced DBS will be actioned for you (unless you are on the DBS Update Service).

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week.  Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
