Job Introduction
Home 1st Reablement provides short term support to adults who have been discharged from hospital or are recovering from a period of ill health. The team are based at Bletchley Community Hospital and sit alongside health colleagues such as nurses and physiotherapists. We work with people for up to 6 weeks to encourage them to regain skills in areas such as washing and dressing, mobility and preparing meals.

Main Responsibility
- To provide reablement support, including personal care, to adults in their own homes, following agreed care plans and goals.
- To assess and review the needs of individuals, working with them to identify their goals and any risks that may arise.
- To take responsibility for altering and distributing work schedules, ensuring that all visits are covered and support workers are supported in their role.
- To ensure records are kept and maintained in an accurate and timely manner.
- To ensure any concerns are escalated to a line manager as soon as possible.
The Ideal Candidate
- Hold a Level 2 Diploma in Social Care (or be prepared to work towards this) or equivalent qualification/experience.
- Will have experience of delivering care support in a community setting.
- Will have a practical approach to resolving problems as they arise.
- Will have good IT skills and an attention to detail.
- Will have excellent communication skills, both written and verbally.
- Be able to work a variety of shift patterns from 06.30 – 22.30 hours over a 7 day rota.
- Will be a car driver and have access to a vehicle (or driver).
