Job Introduction
We’re seeking Relief Support Workers to join our well-established Reablement Team. We provide short-term home care and reablement services to help people remain independent. This includes support with their personal care needs.
You’ll work alongside our Occupational Therapy Team and partner therapists (part of CNWL - Central and North West London) to identify which aids and equipment would help people achieve their goals, and help them to use these effectively.
The Reablement Service operates between 7am and 10:30pm and this role covers shifts on an ad hoc basis, including evenings, weekends and bank holidays. As it’s also community-based, you must have a full UK driving licence and access to your own vehicle.

This is a site-based role.
Interview for this role will be held on 3 and 4 March.
Main Responsibility
- Deliver high-quality reablement services to people with medium and complex support needs, providing them with empowering, person centred care and support whilst they recover from a period in hospital or episode of ill health.
- Effectively record information in accordance with service requirements, this will include contributing to the writing of risk of assessments and individual support plans.
- Using MKCC IT systems effectively as well as giving appropriate verbal handover and feedback to colleagues, external agencies and families as appropriate.
- Complete all mandatory training and any additional training identified as required, this will include safeguarding vulnerable adults and supporting with medication.
- Using the learning achieved to improve own practice and inform and actively participate in service development and improvement.
- Provide intimate personal care to vulnerable adults, ensuring their dignity is maintained and following infection control guidance, this will include using personal protective equipment, moving and handling and the use of mobility aids.
- Provide flexible support across the service, this may include working a flexible working pattern, working from different service areas, in the community and in the homes of people using the service.
- Ensure maintenance of highest standards of care and working to agreed health and safety policies and care plans at all times.
The Ideal Candidate
- Flexible as you might be asked to work in another provider services.
- Evidence of excellent communication skills, including good writing skills and ability to record clearly, accurately and concisely.
- Demonstrate the ability to respond to non-verbal communication, and awareness of own behaviour and impact.
- Demonstrate an understanding of the importance and purpose of confidentiality in recording and sharing information.
- Evidence the ability to respond in complex or crisis situations, with awareness of assessing and managing risks.
- Access to own transport to work across different locations in Milton Keynes is essential.
- For this role, the candidate is required to have a full UK driving licence, along with access to a vehicle. Evidence of the driving licence and vehicle insurance with business insurance will be required from the candidate should they be offered this position. Without this documentation the candidate will not be appointable.
- Should you be offered the role, as part of the pre-employment checks an Enhanced DBS will be actioned for you (unless you are on the DBS Update Service).

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week. Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
