Rehabilitation Support Worker

Job Introduction

We’re seeking a compassionate and proactive person to join our Reablement Team on a six-month fixed term contract. 

 

The team gives high-quality care to people recovering from a hospital stay, injuries or ill health. 

 

You’ll mainly work in the community, supporting residents with varying needs as they regain independence through therapy-led care. You’ll keep accurate records and will work collaboratively with families and professionals. 

 

This is a hands-off, eyes-on role focused on enablement, dignity and safeguarding, with opportunities for training and development through our care academy. 

 

This is a site-based role. 

 

Interviews will be held w/c 13 October.

Main Responsibility

  •  Deliver high-quality, therapy-led recovery and rehabilitation support to individuals with varying levels of need following hospital discharge, injury, or ill health.
  • Accurately record and update care information, including risk assessments and support plans, using MKCC IT systems, EMAR, duty notes, and work logs.
  • Complete all mandatory and additional training through the care academy, applying learning to improve practice and contribute to service development.
  • Provide responsive care in line with individual plans, identifying and reporting changes in health or mobility, and following safeguarding procedures.
  • Promote independence and enablement through a hands-off, eyes-on approach, supporting daily living tasks and recognising when additional help is needed.
  • Deliver personal and intimate care with dignity, following infection control and health and safety policies, primarily in service users’ homes and occasionally in hospital settings.

The Ideal Candidate

  • Experience in delivering person-centred care and rehabilitation support, ideally in community or hospital settings.
  • Confident using digital care systems (e.g. EMAR, duty notes, support plans) and communicating effectively with colleagues, families, and external agencies.
  • Committed to ongoing learning and development, including completing mandatory training and engaging with the care academy.
  • Able to respond to changing needs and risks, with a good understanding of safeguarding and crisis support procedures.
  • Passionate about promoting independence through a hands-off, eyes-on approach, while recognising when additional support is needed.
  • Comfortable providing personal care with dignity, following infection control and health and safety protocols. 
  • Able to manage deadlines and perform under pressure while maintaining accuracy and attention to detail.
  • If you are appointed to this role, it is subject to an Enhanced DBS check as part of the pre-employment process. Along with Safer Recruitment requirement.

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week.  Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
