Home 1st Coordinator

Job Introduction

Home 1st Reablement provides short term support to adults who have been discharged from hospital or are recovering from a period of ill health. The team are based at Bletchley Community Hospital and sit alongside health colleagues such as Nurses and Physiotherapists. We work with people for up to 6 weeks to encourage them to regain skills in areas such as washing and dressing, mobility and preparing meals.

Main Responsibility

  • To provide day to day leadership of a team of reablement workers who support adults in their own homes.
  • To support in undertaking assessments and reviews to ensure the service meets the needs of individuals.
  • To monitor the quality of support given to ensure it consistently meets Care Quality Commission (CQC) standards.
  • To lead on the management of staff absence and performance as necessary.
  • To respond to and investigate service user concerns and ensure issues are escalated to the Registered Manager.
  • To ensure all contacts are accurately recorded on departmental IT systems.

The Ideal Candidate

  • Will have a minimum of 3 years experience in supporting vulnerable adults.
  • Have a sound knowledge of CQC requirements and legislation such as the Care Act.
  • Be able to remain calm and make balanced decisions in complex situations.
  • Will preferably have experience of managing a team and supporting staff.
  • Will have good IT skills and an attention to detail.
  • Will have excellent communication skills, both written and verbally.
  • Be able to work a variety of shift patterns from 06.30 – 22.30 hours over a 7 day rota. 
  • Will have a positive attitude and thrive in an area that is always adapting to changing needs.
