Job Introduction
As a Home1st Reablement co-ordinator, you will be responsible for participating in the Triage process, receiving, and assessing referrals for the Reablement team. You will undertake assessments with the service user to identify Reablement needs and plan. You will carry out spot checks and audits.
Main Responsibility

To lead and supervise a team who work directly with service users to provide a high quality and responsive reablement care service. To promote independence, choice, rights and dignity of service users in community and with reablement service.
