Job Introduction

The Adult Social Care - Access Team are looking to recruit a new Customer Liaison Officer.

We are looking for an enthusiastic and motivated individual with developed customer service skills. As part of the Access Team, you will be the first point of contact for people making enquiries about Adult Social Care. This team provide customer service to the public and professionals contacting Adult Social Care by telephone, post, email or online referrals.

This team have an important role in making sure that individuals who contact them understand how the care and support system can apply to them, and provide information or advice and signposting to other sources of support or information, as well as to offer/consider prevention and wellbeing services.

The Access Team is a busy and fast paced team where every day is different. We are looking for someone who enjoys a challenge and has knowledge of Adult Social Care.  If you are interested in making a difference and want a varied and rewarding role, then this is the job role for you.

Main Responsibility

  • Identifying customers’ needs for services through robust collection of information and using professional judgement on how to progress the referral.
  • Gathering relevant information about the person/carer, to enable the Access team to provide appropriate advice and refer onto other agencies.
  • Helping the person/carer to look at what support might be available in the community, or their support network and provide a solution focussed response in the form of information, advice and signposting.
  • Identifying if someone appears to have care and support needs, or a carer who appears to have support needs and offer a needs or carers assessment if appropriate.
  • Having a good understanding of Adult Social Care and services in the community both statutory and voluntary.
  • Using electronic database systems to input and retrieve data and information whilst dealing with enquiries.
  • Ensuring accurate recording within Adult Social Care systems to support the decision making process.
  • Contributing to the maintenance and accuracy of the information stored.

 

 

 

 

The Ideal Candidate

  • Someone who enjoys talking to people and trying to put things right.
  • Someone who has good customer service skills.
  • Someone who knows about, or can quickly learn, the Adult Social Care System and community based services.
  • Someone who can use our computer-based systems to search and record information.
