Job Introduction

We’re seeking an enthusiastic and motivated person with excellent customer service skills to join our busy and fast-paced Access Team within Adult Social Care.

The Access Team play a vital role in making sure that people who contact us understand how the care and support system can apply to them and provide information, advice or signposting to other sources of support. We also offer/consider prevention and wellbeing services using a strength-based approach.

As a Community Access Advisor, you’ll be the first point of contact for people making enquiries about Adult Social Care by telephone, post, email or online referrals so good communication skills are essential.

Main Responsibility

  • Provide information, advice, and signposting by connecting with local resources and explore innovative solutions for prevention strategies.
  • Complete referrals to internal and external organisations via telephone or written communication and on occasion, the need may arise for face-to-face interaction with organisations.
  • Research, share and maintain information regarding community services.
  • Maintain accurate records while ensuring an adherence to data protection and confidentiality statements.
  • Capture and analyse the information to ensure that all conversations are recorded and managed in an effective and timely manner.
  • Recognise and manage potentially emotional, stressful and complex situations in relation to callers, where there may be barriers to communication, including those who exhibit signs of confusion, aggression, distress, or violence.

The Ideal Candidate

  • Enjoys talking to people and trying to put things right.
  • Good customer service skills.
  • Knowledge of, or can quickly learn, the Adult Social Care System and community-based services.
  • Able to use our computer-based systems to search and record information.

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week.  Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
